Research with Impact: the South London Stroke Register (SLSR) research programme

We are blogging about how the South London Stroke Register (SLSR) and other stroke research at King’s College London has generated new insights into:

  • risks of stroke
  • effectiveness of stroke care
  • how well effective care is implemented
  • impacts for stroke survivors.

The SLSR has provided data and analyses that informed a Parliamentary Public Accounts Committee report on stroke, the National Stroke Strategy, and contributed to two National Audit Office reports on acute and longer-term stroke care. The latter contributed to a major service reconfiguration in London, which has led to lower mortality and more efficient use of health care resources for stroke. The SLSR provides a platform for designing and evaluating new models of stroke care, including the largest trial of Early Supported Discharge, a cost-effective intervention which is now provided in the majority of hospitals in England, as well as being rolled out internationally.


The Burden of Stroke in Europe report: need for more support for more stroke survivors.

BoS coverThe number of strokes across the UK could rise by almost half (44 per cent) (i) in the next 20 years, according to a new report (ii) by researchers from King’s College London. The expected increase is due to our ageing population. The chance of having a stroke increases as we get older.

The increase in strokes means that the number of people living with the effects of stroke in the UK could rise by a third (32 per cent) by 2035 (iii).

The researchers cannot be sure about the predicted size of the increase, partly because the data on how many strokes happen and how many people survive stroke is not perfect.

The quality of hospital care for stroke patients varies widely across Europe. The researchers also found that, in general, countries in Europe do not collect (or publish) much information on what care stroke survivors get after they leave hospital. The data that is available shows that between countries and within countries (including the UK), there is a lot of variation in the care and support offered to stroke survivors. The researchers conclude that there needs to be more of a focus on long-term rehabilitation and support.

Predicting functional recovery after stroke

Clinical prediction of a patient’s recovery is an important medical aim for stroke survivors.

A team including researchers from King’s College London developed and checked (validated) a tool that seems to accurately predict how well or poorly a patient will recover after their stroke. The team used data from the South London Stroke Register  to develop and check the tool. The tool could help clinicians to work out the likely recovery of a patient based on various factors such as their age, sex, level of disability and stroke subtype. They could use this information to provide more personalised care for patients.

Air pollution research and stroke

Long-term exposure to high levels of outdoor air pollution has been linked with poor health in many cities around the world.

Using information from the South London Stroke Register (SLSR), a team including researchers from King’s College London wanted to find out if people who had a stroke were affected by higher levels of outdoor air pollution. They looked at the health of 1800 people who had a stroke between 2005-2012.

The researchers found that those living in areas with lower levels of particulate matter live longer after a stroke. Particulate matter is a type of air pollutant made up of tiny bits of matter floating in the air from natural sources such as dust and pollen as well as man-made sources like soot, smoke and car exhausts.

SLSR researchers and air pollution scientists from King’s College London are doing more research to find out how particulate matter causes poor health effects in people.

Long-term outcomes: survivors’ experiences up to 15 years after stroke

Researchers at King’s College London use South London Stroke Register data to understand the long term consequences of stroke. Recently they did an analysis of outcomes for Register participants who lived up to 15 years after their stroke. The outcomes they looked at were survival, disability, activity, cognitive impairment, quality of life, depression and anxiety.

The researchers found that one in five people live at least 15 years after their stroke, and that many of these people live with disability and psychological problems. For example, one in 10 of the people who lived for 15 years after their stroke had lived with moderate to severe disability since their stroke.

The study emphasises that, as more people survive stroke, “research and health services will need to increasingly focus on preventing and managing the long-term consequences of stroke”.
